Need help on how to declare goods?

Anybody here has ever make declaration letter when you deliver goods to overseas/Australia?

I need to create a letter as declaration for my goods but i do not know what need to list like my name,details etc etc?

u shipping by?

if using courier services, they normally have a slip or u can print out over the net the form for declaration. even for singpost they have a slip to fill in.
