Flash Head Spoilt

Anyone know what are the causes to make the flash head spoilt? I'm using canon 420EX, then one day my flash spoilt while the flash still can able to flash the light but the photo came out to be dark instead of "flashed" photo...

Does it happens in TTL or are you using manual? Are you sure it is not sync problem?

what is the shutter speed you set?
