How to take this kind of photo?

Not too difficult. You need a med-tele lens. Pose the model at far, focus on the model face, use aperture about f4 or f5.6, then in front of your lens, you hold a bunch of leaves very near to your lens. The green leaves will be out of focus due to DOF control, and becomes blur.
Alternatively you can take a sharp photo and use photoshop to blur the leaves.
More alternatively you can come join us again, :D

To add to what austin had mentioned, the photographer seem to had use a bit of fill in flash too.

There is a small chance the photographer is standing near a dead end on his left, possibly leaning on the wall. visible vignetting on the top left hand corner of the picture.

Personally, I perferred the last picture where she is in red sitting. Made full use of natural lighting with the sun from her back, something I had yet to get the hang on properly :D .
