E-TTL should compensate for the difference in flash power to achieve the ratio you are looking for. But it may not be that consistent if your subject is moving or you're changing angles frequently.

I think so, this appears to related to Newton's rings type of interference pattern. I would think front element design plays a part if it is very close to the filter glass and very slightly spherical.
