I like the lighting, but it does not give me a sense of helplessness or foreboding.

Flash is too harsh resulting overexposed on the on the right portion of the body. Try setting up your flash at other angles to experiment. Subject's expression is in a sense 'normal' to give the viewers the helplessness you want to portray.
