Use PhotoShop 7's Healing Tool. The best you can get.

Lightbulb Grain Surgery v2 [smooth skin surface]

    Plug-in : http://www.pluginsworld.com/Photoshop/plugins.php3?action=editor79&soft=Photoshop

Grain Surgery is a set of intelligent noise manipulation tools that plug-in for both Adobe After Effects and Photoshop (Win and Mac).
Grain Surgery's "Remove Grain" filter is a clear breakthrough in noise reduction. Its advanced algorithm does not rely on blurring or averaging and gives very clean results with minimal loss of sharpness and detail. It also cleans up compression artifacts and halftone patterns. "Add Grain" lets you generate exactly the grain you want, or draw on an extensive library of commercial stock presets. With "Match Grain", you can extract the noise model from one image or layer and precisely apply it to another image. You can even compensate automatically for the grain already present in the target image.
Grain Surgery is essential for tasks such as image compositing, enlargements, image restoration, descreening and integration of digital and photographic elements. It is useful in any medium, from art prints to large-scale posters, and from print publishing to Web streaming
