Look at other people's product shots, e.g. in magazines, etc.

Depends on what product.. more likely, you'll need macro lens.

Again, no hard and fast rules. Refer to product shots and you will see that there's the common clean backgroud, and there's also the non-so-clean backgruond with complementary products.
