I had a similar problem on my Manfrotto Quick release. Tighten using an allen key. Luckily my whole setup did not drop off while I was carrying it using the tripod only. The loosen part is the Quick release lever for my case.

I didnt notice it until I was cleaning the Gears. I thing its size 4mm. I have a spare one if you want.

Is it the quick release catch loose?
There is only a 2.5mm allen screw to tighten.

    4mm screw and 2.5mm allen head .

Loctite is a must to prevent it falling off in future.
