the noise is due to high ISO, I like to use ambient light with fill flash as much as possible, notice the nice soft shadows? The noise does not bother me, i feel that good light with noise is always better than bad light without noise. but it is your choice

Some people do, but not me, I shoot in S mode with shutter speed set at 1/160 or 1/200.
also the use of proper camera holding technique with breathing control and as much support as possible.

if that cannot work then, I guess you have to use a tripod/monopod
