Sometimes mounted, sometimes un-mounted.

Usually un-mounted after cleaning. But if planning to shoot with same lens next couple of days, I leave it mounted.

Caps mostly on. Off if the camera was brought out in rainy weather.

Lens cap off so that moisture that is trapped in between 1st element/filter and lens cap can be released.

Default store

same as nod3
All Lenses unmounted with filter on and front lens cap and rear lens cap on.
For camera bodies always with body cap on.
Lens cap and filter are not airtight, so vapour (if any) will be able to escape even if they are on.
If you have sweaty palms ( I don't have) always wipe your equipment, lens barrels, camera bodies before you store or else there may be corrosion.
I have seen some horrid stuff before.

Default Lens cap always on

I always leave my lens caps on. Body cap on if dismounted. Even dry cab has dust sometimes.

You are right: There are always gaps between cover and lens for moisture to leave. So leaving cap on keeps dust out but allows moisture to leave.

Its not air-tight, the humidity level around the front element/filter will be the same whether the cap is on or off. For that matter, lens bodies aren't air-tight either, otherwise how does fungus get to the middle glass elements?
