Battery pack in the pocket. Camera and flash on a table. Walk away pulling the cam and flash. Cam body ok. Lens and flash broke into half.
So far, after so many years, only lost a Tk12-24 lens hood, my D300s bottom rubber cover for the grip contact, the rubber eye cup.
Other wise nothing serious so far.
BTW, I have yet to scratch or damaged any protective UV filter of my lenses. That being the case, I might consider forego the filter.... But, without filter, somehow, it feels like riding a bike without helmet, that is without protection. Well, the filter is just to feel safe la.

Nothing too disastrous for me, just a ND filter fell and rolled off a jetty into the water.
My Cam was mounted on a tripod with 24-105L, getting ready to shoot the laser show at victoria harbour in HK. My first trip to HK and it was my first day. Entire tripod collapsed , lens glass shattered .... but unusually, I was damn calm...LOL
