Hands itchy, change my kit lens with my boss' lens. The whole thing tilted over the rail guard, and almost drop into SG river. Lucky the cam strap was still around my neck.

That was the day I retire the Nikon free tripod.

Event: Pacu Jawi (post harvest Bull Run festival)
    Year: 2009
    Location: Outside Padang
Situation: see image below (courtesy of Maxby Chan from Malaysia) ... padi field filled with water totally mixed and blended in BS (literally) and pee of 100+ bulls
A bull bumped me and I was sent diving into the cess pool
The camera still works and the humble 50/1.8 is still taking great pics today ....
But the flash trigger from China KO'ed

    Conclusion: Never Again

Nearly drown my Canon 400D with Tokina 10-17mm fisheye during a Scuba Diving trip this Feb... in a rush to get ready, didn't install the water proof O Ring properly... luckily tested the whole setup inside the washing tank and not in the Sea... notice the water inside the housing immediately, but the pooling water didn't manage to touch either the camera or the lens....

Once after a street shooting session alone, I put my camera (with lens attached) inside my Crumpler 7MDH bag - along with other items.

Arrived at MRT station, grabbed my EZ-link card, and shocked to find my lens cap in my pocket.

Luckily the nothing scratched the lens glass

At least you got the shot.
